AZOVA Telemedicine Provider Network
Participate in AZOVA's Paid Telemedicine Programs and get reimbursed for each visit that you complete. The average video visit on AZOVA is 7-9 minutes. The average messaging visit takes 3-5 minutes to complete. Virtual primary care visits are more comprehensive and take more time. You will be paid on a monthly basis for each visit you have completed in the previous month. AZOVA provides in application virtual medical assisting services to assist you in patient care. You can request assistance with patient care from the virtual medical assisting team within AZOVA. AZOVA offers telehealth appointments that are scheduled, next available and on demand. Scheduled appointments: Contractor agrees to set hours of availability within provider’s account and to be available at the times that are set by Contractor. Patients will select an available time and schedule themselves. Contractor also agrees to be available for next available and on demand appointments. AZOVA will strive to send next available appointments during the hours that Contractor has set for scheduled availability and Contractor agrees to complete these appointments in no less than two hours from the time the appointment is received when the appointment is sent during Contractor’s available hours. AZOVA also offers On Demand services. On Demand services enable the patient to choose an appointment type for the next available on demand provider. AZOVA may send On Demand appointments at any time. On Demand appointments must be claimed prior to completion and will be allocated to the first provider who claims the On Demand appointment. Contractor will not claim an On Demand appointment and not complete it.
AZOVA Virtual Primary Care Network
As a member of the virtual primary care network on AZOVA, patients can choose Contractor as their primary care provider. AZOVA will provide a branded and personalized online clinic featured on the AZOVA website and which you, Contractor, can use to promote Contractor’s online practice, where patients can learn about Contractor and Contractor’s practice style and areas of expertise. Patients can choose a new patient primary care visit to establish care and then follow up online through any of the condition specific appointments that Contractor also offers on AZOVA. Patients may access Contractor’s primary care services through a membership or an individual visit. AZOVA will provide virtual medical assisting services and will assist in coordinating lab testing, prescriptions and imaging as needed. As the patient’s virtual primary care provider, Contractor will be responsible for providing primary care services including management of chronic conditions such as diabetes, hypertension, asthma and other common conditions. Contractor is responsible for assessing each patient’s compliance with screening exams, vaccinations and lab testing and for making recommendations to each patient based upon the standard of care for each patient based upon influencing factors including gender, demographics and other comorbidities. A virtual medical assistant team is available to assist Contractor in follow up and communicating with Contractor’s patients. Contractor will be responsible for writing prescriptions, lab orders and imaging orders for Contractor’s patients. AZOVA Care Group will provide recommendations for age appropriate screenings and lab testing. Contractor is responsible for patient care and in implementing the standard of care with each patient.
General Medical, Men’s Health, Women’s Health Condition Specific Services
Condition specific telehealth services are focused on a specific condition. Contractor agrees to respond to and complete each patient visit within the same calendar day or no less than 24 hours from the time the appointment is received.
Medication Refill appointment
AZOVA's medication refill appointment is a very quick medication visit that enables patients to request a one to three month refill of one or more specific medications. Contractor agrees to review the patient’s provided medical history, medication list and allergy list and to exercise clinical judgment when choosing to refill medications.
Lab Order and Imaging Order appointment
AZOVA’s lab order and imaging order appointment enables a consumer to request a prescription for a specific lab test or imaging study. Contractor agrees to review the patient’s provider medical history and clinical presentation and to use clinical judgment when choosing to order the requested laboratory test(s) or imaging studies.
